To obtain your Low Irritation Pledge Refund, just follow the following steps:

  2. Have your physician sign and date the form and provide his/her physician license number, certifying that you had to stop using NeoBenz Micro due to irritation.
  3. Completely fill out the rest of the form, including your signature and contact information.
  4. Send this original form, along with your original receipt for NeoBenz Micro (one carton of NeoBenz Micro Wash Plus Pack, one tube of NeoBenz Micro Cream or one box of NeoBenz Micro SD), showing the drug name and your out-of-pocket costs as proof of purchase, to the address below, and no later than 30 days from date of physician's signature. No photocopies will be accepted. Please retain a copy for your records.
